My Quality of Life Survey
What is a My Quality of Life Survey? The My Quality of Life survey is a survey completed online through survey monkey. The goal of the survey is to make sure that the services you receive give you the opportunity for independence in making life decisions, to fully participate in community life, and to make sure your rights are respected.
The survey takes the Home and Community Based Services Final Rule and breaks it down to show your life choices.
