DESCRIPTION:Cass Family Clinic just launched the Mobile Health Clinic to better serve the residents of Southwest Michigan. The year-round Mobile Health Clinic offers flu shots\, urgent care\, primary care\, patient education\, blood pressure checks\, health and wellness exams\, sports and well child physicals\, and other healthcare needs. \nOur goal is to ensure that residents have access to quality healthcare–no matter where they are. The Mobile Health Clinic allows us to deliver care in a new and innovative way\, reaching patients who might otherwise face challenges accessing services. \nThe Mobile Health Clinic services are available to anyone in the community\, not just Cass Family Clinic patients. Residents may register as a Cass Family Clinic patient at the Mobile Health Clinic. Medicaid and most other insurance plans are accepted. DESCRIPTION:Our Mobile Market Program was created in an effort to bring healthy foods directly into low-resource communities\, eliminating travel burdens and barriers\, and improving access to essential food items. \nThis initiative acts as a “pantry on wheels”\, offering a selection of meal kits of nutrient-rich pantry staples\, accompanied by simple\, easy-to-follow recipes. In addition\, neighbors can select protein and dairy products\, fresh fruits\, and vegetables to take home with them as well. \nMobile Markets are tailored to serve rural areas\, senior assisted living facilities\, low-income housing communities\, transportation-limited neighborhoods\, and school programs. Our goal is to close food access gaps\, provide nourishment to families\, promote nutrition education\, and strengthen our community by addressing food insecurity. \nA sponsorship of a Mobile Market Program allows South Michigan Food Bank the opportunity to expand to more locations\, enhance our food options\, and support our neighbors facing food insecurity. Please note that schedules may change. \nYou can also find this information—and many other food resources—in map form on our Resource Map. If you have questions about a specific Mobile Pantry\, please contact the host agency. For more information about client rights while visiting an agency partner\, see our Client Rights Statement here. \nMany Mobile Food Pantry sites will allow you to pick up food as a proxy (stand-in) for another household that cannot physically attend. You will need to be prepared to provide the name\, state of residency\, and household size of any other family you are picking up for. Most Mobile Pantry sites have a limit on how many households you can pick up for and should tell you that limit if you ask.
